Do I need to pick a Primary Care Provider (PCP)?

With a C-SNP plan, you need to pick a PCP.

Do I need a referral to see a specialist?

With a C-SNP plan, you need a referral to see a specialist out-of-network.

Can I choose a separate Part D plan with my C-SNP?

You cannot choose a separate Part D plan with a Medicare Advantage C-SNP plan since pharmacies are part of the network.

What benefits do I get with a Wellcare C-SNP?

With a Wellcare C-SNP plan, you can select a Primary Care Provider who will get to know your health needs, coordinate your care with other providers and develop a personal care plan to help you manage your condition.
